NAE4-HA Distinguished Service Award (DSA)

Overview

This award is given to any current member of NAE4-HA who has paid dues (for at least the immediate two consecutive years prior to the current) provided the individual has served seven (7) years or more in Extension youth programs.  States may recommend one DSA for every 50 paid members or part thereof (i.e. 1-50 members = 1 DSA; 51-100 = 2 DSA; 101-150 members = 3 DSA, etc.)

Eligibility (Who can apply / be nominated)

Members recommended must be actively engaged in Extension youth programs as of the date the selections are made. 

Selection Criteria (How recipients will be chosen)

The state or unit selection committee may use the following score card as a guide in making the final decision.  (Distinguished Service Award may be received only once.):
A.  Professional Attitude (15 Points)
Work with Professional Association
Extension Committee Work and Responsibilities

B. Professional Improvement (15 Points)
Advance Formal Education
Supplemental Training (Attendance at National Meeting can be counted)

C. Professional Accomplishments (50 Points)
Size and Depth of Program
Program Improvements
Program Innovations

D. Personal, Community (15 Points)
Personal Attitudes and Interests

E. Special Honors (5 Points)
Professional Honors
Community Honors

Total 100 Points

Packet Requirements (What to submit)

DSA attachments will include:
A completed NAE4-HA Member Recognition Application

A brief summary of program accomplishments of not more than 50 words to be printed in the program book.

A 20-word bio to be read during the awards presentation at Conference.

A head and shoulders digital photo submitted in .jpg format. The file name will be the applicant’s name and state.
DSA submissions (A, B, C, and D above) will be submitted electronically in MS Word format to State Contacts. Selection is made by state/unit. Late or incomplete entries will not be recognized.
